Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during you next office visit.Return to Top

Your MyChart information comes directly from your electronic medical record at your doctor's office. You may request to have your record amended any time you feel that information in your file is incorrect. Contact the Medical Records department at 630-859-7266 and request an amendment form. Complete the form and return it to Dreyer Medical Clinic, 1870 W. Galena Blvd, Aurora, IL 60506 Atn: Medical Records. Your request to amend your record will be completed within 60 days. A letter of the acceptance or denial of your request will be mailed to you.Return to Top

Yes you can. This is called Proxy access.
For pediatric patients this allows a parent (or guardian) to log into their personal MyChart account, and then connect to information regarding their family member. Complete a Proxy Consent Form at the time of the childís visit and you will receive the proxy information via U.S. Postal mail. Note: At age 12 this privilege is automatically changed to a more restricted access.
For adult patients this allows another person, of the patientís choosing, to log into their personal MyChart account, and then connect to information regarding the patientís information. The patient will need to complete a Proxy Consent Form at the time of their visit and the proxy information may be given to the patient or the proxy at the time of the visit. This privilege can be revoked by the patient at any time.Return to Top
No, only questions regarding your health should be asked from your MyChart account. MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.Return to Top
No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.Return to Top

Contact your doctorís office or at your next visit a new activation code may be requested. If calling a new code will be sent via U.S. Postal Mail. Privacy issues prevent us from e-mailing a new access code to you. If in the office the new code will be handed to you.Return to Top

We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 20 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 30 days).
